Try the Pomodoro technique.

Plan ahead while giving yourself rewards and consequences. This needs to be something you do for yourself, instead of your parents or teachers making you do things.

Choose a reward to give yourself that would make getting something done worth it. This reward needs to take no more than 4 minutes long.

Give yourself different types of breaks and rewards depending on how much you get done in 25 minute increments. For example, 1st break is checking texts, 2nd break is snack time, 3rd break is a video game. (By then, you've worked an hour and a half.)
